Job Description:

Role Purpose: Plan and provide care for patients with potential skin care problems and those with wounds and/or stomas. WOCN/WCC will consult with other health care team members in the management of acute and chronic wounds and/or ostomies.

• Effectively collaborates with other care providers and acts as consultant for patient care management of ostomy, wound care, and continence using best care practices. WOCN/WCC meets this goal by seeing inpatient consults within twenty-four (24) hours of admission during working hours and seeing patients in the clinic setting within thirty (30) minutes of their scheduled appointment time. WOCN/WCC exceeds this by assisting with other's cases and continuing education.
• Effectively educates hospital staff regarding best practice care for wounds and ostomies to ensure appropriate care is provided to all inpatients. WOCN/WCC meets this goal by discussing best practice care and having staff nurses do return demonstration of care with the WOCN/WCC member present to answer any questions whenever possible. Educating CNA's on proper turning schedules and skin care. WOCN/WCC exceeds this goal by participating in annual CNA and RN skills days as well as development of teaching plans for these activities.
• Effectively educate patients and their family members/caregivers about wound and/or ostomy care for patients who will need to continue care at home. WOCN/WCC meets this goal by discussing care with patients and their family members/caregivers followed by return demonstration by patient/family/caregiver to ensure that proper wound care has been learned and to increase likelyhood of wound care retention. WOCN/WCC meets this goal by instructing patients/family members/caregivers about obtaining supplies needed for care at home. WOCN/WCC exceeds this goal by performing telephone follow-ups with patients/family members/caregivers at home.
• WOCN/WCC assists physician in providing care. WOCN/WCC meets this goal by collaborating with physician regarding care, assistance in obtaining needed supplies for physician's use, and assisting with procedures. WOCN/WCC exceeds this goal by discussing continuing education for improved techniques and new products with the wound healing team.
• WOCN/WCC administers prescribed medication orally, topically or via injection as ordered by physician. WOCN/WCC meets this by administering medications following established policy and procedures.
• WOCN/WCC documents all patient data accurately and timely. WOCN/WCC meets this goal by charting on each patient within twenty-four (24) hours of seeing patient in clinic and documents in progress notes at time of evaluation of inpatients. WOCN/WCC exceeds this goal by assisting with improving documentation in clinic through continuing education.

Job Requirements:

Minimum Qualifications:
• Associate’s in Nursing
• Registered Nursing License

Preferred Qualifications:
• Bachelor’s in Nursing
• WOCN preferred
• Hyperbaric

Physical Requirements: Employees in this position are, or can be, required to possess the following physical capabilities:
• Lifting, carrying, pushing and/or pulling up to 100 lbs.
• Stooping, kneeling and/or crouching daily.
• Standing or walking more than 8 hours a day.
• Running to respond to patient emergencies.
• Reaching, grasping, fingering, and/or feeling more than 7 hours a day.
• Visual acuity in order to correctly distinguish skin tone variations, evidence of IV infiltration, infection; able to read telemetry, blood pressure gauges, etc.
• Audio acuity in order to correctly distinguish various body sounds (i.e., respirations, heart tones, bowel sounds).
